Step 2: Fill Out the TFF Application Form 填写TFF申请表

Business Information (商家信息):

• Business Name, Business Address, Contact information
• Provide your valid business license and any relevant health permit information.

• Business Name (商家名称)、Business Address (商家地址)、Contact Person (联系人)。

Event Information (活动信息):

• Event Name: Moon Festival  
• Event Date: October 5, 2024  
• Event Time: 10 AM – 4 PM

Temporary Food Facility (TFF) Information (临时食品摊位信息): 

• Booth Name: Provide the name of your booth (e.g., Zhang’s BBQ).  
• Booth Type: Describe the booth type, dimensions, and setup (e.g., enclosed booth, canopy, etc.).  
• List of Food/Beverage Items: List all the food and beverages you will be serving, including condiments and ingredients.

• Booth Name (摊位名称)：填写您的摊位名称（例如：张三烧烤）。
• Booth Type (摊位类型)摊位尺寸及设置方式：描述摊位是否为封闭摊位、遮阳篷，摊位尺寸，设置方式等细节。
• List of Food/Beverage Items (食品/饮料列表)：列出您将在活动中提供的所有食品和饮料，包括调味品和配料。

Advance Preparation/Storage Activities  (预先准备/储存方式): 说明食品是预先准备好还是在活动现场准备

• Will food be prepared in advance? If yes, provide the name, address, and dates of use for the approved kitchen.  
• If you use a co-packer and receive food a day or more before the event, detail the food preparation/storage activities (e.g., “Food will be stored in the approved kitchen and transported to the site on the morning of the event”).

• Will food be prepared in advance? (食物是否是提前准备好的？) ：如果是请填写您使用的厨房的名称，地址和使用厨房的日期和时间，需要是Approved Kitchen (已批准的厨房)
• 如果您使用共同包装商（co-packer)，并在活动开始前一天或更早收到食物，请提供食品准备/储存活动的详细信息：详细的说明食品的准备和储存过程，例如“食品将在批准的厨房中储存，活动当天上午运至现场”。

Food Handling Procedures (食品处理程序):

• Food Preparation Methods: Specify how each type of food will be handled (e.g., cold, hot, room temperature, cooked to order, etc.).  
• Equipment Used for Food Storage/Preparation:** Provide details of all equipment used for storing and preparing food (e.g., refrigerators, heaters, etc.).  
• Temperature Control: Describe how you will ensure that food temperatures meet safety requirements during the event (cold food must be kept below 41°F, and hot food must be kept above 135°F).

• Food Preparation Methods (食品准备方法)：请标明每种食品的处理方式（冷、热、常温、按订单烹饪等）。
• Equipment Used for Food Storage/Preparation (食品存储/准备的设备)：提供您将使用的所有食品储存设备（如冰箱、加热器等）。
•  Temperature Control (温度控制)：提供您将如何确保食品温度在活动期间符合要求（冷食需保持在41°F以下，热食需保持在135°F以上）。

Signature (签名):

• Ensure that your application includes a valid handwritten or electronic signature (e.g., DocuSign).  
• 请确保您的申请表上包含合法的手写或电子签名（如DocuSign）。

Important Note: Handwritten fonts are not accepted.

重要提示：填写文件需要使用电脑填写，不可以手写。签字部分可以手写签字，也可以电子签字，但要注意电子签字不能是手写字体。

Supporting Documents (需要提供的文件资料):

• Food Facility Permit (if applicable): If you already have a Santa Clara County food facility permit, provide a copy of this permit.  
• Out-of-County Mobile Food Facility Permit: If you are an out-of-county vendor, provide a copy of your current out-of-county food facility permit.  
• Food Source Information: If food is prepared before the event, provide the relevant permit information for the preparatory kitchen and its health permit. If you plan to purchase and prepare food on the day of the event, provide a detailed food purchasing plan and food safety measures.  
• Time as a Public Health Control (TPHC) Form (if applicable): If your food will be held at room temperature for more than 2 hours, you must complete and submit the TPHC form.

• 外县移动食品设施许可：如果您是外县供应商，您需要提供您当前的外县食品设施许可的副本。
