Event Waiver

Event: China Central Television (CCTV) Film and Television Media Center National Children’s Art Talents Selection San Francisco

THIS FORM MUST BE SIGNED PRIOR TO PARTICIPATION IN THIS EVENT:

I/My organization wish to participate in the above-described event. I/My organization understand that I do so at my own risk. In exchange for being permitted to participate in the Activity, I/My organization hereby release the BigBang Artwork and Star Alliance Culture Inc., and their all employees, officers, agents, representatives and volunteers (all collectively referred to as the “Released Parties”) of the foregoing from liability for and hereby waive any and all claims for any loss, damage, injuries to person or property, death, claims, demands, lawsuits, expenses and any other liability of any kind, or of to me or any other person, directly or indirectly arising out of or in connection with my participation in or attendance at the event. I/My organization further agree to hold harmless, indemnify and reimburse the Released Parties from and for any sums, costs, or expenses incurred by any of the Released Parties or paid by them to any person (including me or my insurers) in connection with any accident loss, damage, injury to person or property, or death sustained by me or others in connection with my attendance at or participation in the Activity. This means that I/My organization will reimburse the Released Parties if anyone makes a claim against them based on damages or injuries to person or property I/My organization may suffer or cause. I/My organization understand that as I/My organization am requesting such instruction, that I/My organization willingly waive and release all future claims against them as stated above. Further, I/My organization agree to abide by all event policies, state, local, and federal laws for the duration of the event in which I/My organization am voluntarily participating. I/My organization understand that failure to abide these expectations may result in my/my organization immediate removal from the event, and appropriate or legal consequences. As a participant I/My organization also agree to abide by the event rules, as stated in the event venue Policies and Procedures. Having read and understood the above, I/My organization freely sign this waiver, release and hold harmless agreement. 

Consent of the Parent or Guardian I give consent for my child to participate in the above Event, and I execute the above liability release on my child’s behalf. I promise that I will be accompany, attend and supervise my child as Parent or Guardian when the Event requires.  If any accident incurs, the Event’s organizer shall have my full consent to have my client treated by emergency medical personnel, a physician, or surgeon.  It is understood that the Event and its organizers will provide no medical insurance or expenses for any treatment, and that the cost thereof will be at my expense. I have read and understood the foregoing registration liability release and parental consent form, and agree to all of its terms and conditions.
